Death Certificate Registration (Reporting death of Thai nationals who died in Hong Kong or Macao)
Required Supporting Documents
- Application for Death Registration Form (Click Here)
- In the case that the death occured in Hong Kong: Hong Kong Death Certificate with a Notary Public certified copy
; or
In the case that the death occured in Macao: Macao Death Certificate in Portuguese with an Notary Public certified English translation copy - The deceased’s Thai Passport
- The deceased’s Thai National Identity Card
- The deceased’s Hong Kong, Macao, or other National Identity Card
- The deceased’s Thai House Registration
- The deceased’s Thai, Hong Kong, Macao, or other Marriage Certificate
- The deceased’s Thai, Hong Kong, Macao, or other Divorce Certificate
- Medical records or official government documents stating the cause of death
- Other relevant official documents issued by Thai or local government authorities
- The applicant’s (informant’s) Thai, Hong Kong, Macao, or other Passport
- The applicant’s (informant’s) Thai, Hong Kong, Macao, or other National Identity Card
For items 3-12, the applicant is required to present the original document or a clear, verifiable photograph/ digital copies upon submission. Please note that the Royal Thai Consulate-General does not require and will not retain physical photocopies of the aforementioned documents.
Remarks
- The Royal Thai Consulate-General is responsible for registering deaths and issuing Thai Death Certificates for individuals who passed away within the jurisdiction of Hong Kong Special Administrative Region (HKSAR) or the Macao Special Administrative Region (MSAR).
- Photocopies of Thai, Hong Kong, Macao, or other passports must include the biodata page (showing photograph and personal details) as well as any pages indicating official name or surname amendments.
- Photocopies of Thai, Hong Kong, Macao, or other National Identity Cards must include both the front and back of the card.
- House Registration must be the current booklet format, and the photocopy must include every page containing recorded entries.
- Consular staffs reserve the right to request additional supporting documents or evidence as deemed necessary
